Terms & Conditions*

I confirm that I have read and agree to Katie Palmer Hypnotherapy’s Terms & Conditions, including the payment, cancellation and refund arrangements for my chosen programme.

Health Information & Confidentiality*

I understand that Katie Palmer Hypnotherapy will ask about relevant medical history, physical and mental health, medication and personal circumstances to assess my needs and provide appropriate support. I understand that this information will be recorded and handled confidentially in accordance with the Privacy Policy.

Starting My Programme*

I confirm that I wish to begin my programme on the date agreed with Katie Palmer Hypnotherapy, including if that date falls within the statutory 14-day cancellation period. I expressly request that services begin during this period where applicable. I understand that if I cancel within the 14-day period, I may be required to pay a proportionate amount for services already provided, in accordance with my statutory rights.

Final Confirmation*

I confirm that the information I have provided is correct, that I understand the financial commitment associated with my chosen programme, and that I wish to enter into this agreement with Katie Palmer Hypnotherapy.
